REFERRALS
ⓘ THE CCMHS PROVIDES CANADA-WIDE IN-PERSON AND TELEHEALTH MENTAL HEALTH SUPPORT SERVICES. THE CCMHS DOES NOT PROVIDE CRISIS SERVICES. IN THE EVENT OF A CRISIS, PLEASE GO TO YOUR NEAREST HOSPITAL EMERGENCY ROOM, OR CALL 9-1-1 OR THE TOLL FREE 24H CRISIS LINE AT 1-866-996-0991 FOR IMMEDIATE ASSISTANCE. THE CCMHS TEAM WILL BE HAPPY TO ASSESS YOU ONCE YOUR SYMPTOMS HAVE BEEN STABILIZED.
Who Is Eligible
The CCMHS provides mental health services to competitive and high-performance athletes, performing artists, coaches, and support staff who meet the following eligibility criteria.
Age
Must be 16 years of age or older.
Citizenship
Must be a Canadian citizen or permanent resident.
Participation
Must participate in or be affiliated with a sport or performance domain (ex. dance) at a competitive or high-performance level.
Mental Health
Must be experiencing mental health challenges.
How to Refer
Referrals to the CCMHS are easy. We accept self-referrals from athletes, coaches, support staff, and performing artists as well as external referrals from healthcare practitioners and other allies (e.g., parent, partner, teammate, coach, teacher, physician, sport organization).Referrals can be made online, by email, or by phone. CCMHS CARE Pathway
Service-users (i.e., clients) who are referred to the CCMHS and are interested in the services we offer will go through an intake assessment with our Care Coordinator.
Service-users will be asked to sign a consent form to receive services and allow the CCMHS mental health care team to access and share relevant documents and session notes, permitting the integration and continuity of care on which the CCMHS prides itself. Extended support networks (e.g., members of Integrated Support Team) will only be contacted and/or involved in the care process if service-users wish them to be and if they consent to this.
Service-users who meet CCMHS eligibility criteria will be assigned to a team of practitioners who will provide mental health care. Those who do not meet eligibility criteria will be referred to resources in the mental health or sport community. Providing evidenced-informed and sport/performance-focused care is of the utmost priority at the CCMHS.
